Team/Specialism: Adults Safeguarding Practitioner – Practice, Assurance & Safeguarding Team
Pay Rate: £ per hour (assignment rate)
Summary:
- As a Safeguarding Practitioner you will work as part of the Practice, Assurance and Safeguarding Team ensuring that multi-agency safeguarding systems and standards are implemented across South Tyneside. This includes advising and supporting other teams, partner agencies, care providers and the public in the coordination and management of safeguarding procedures. Safeguarding Practitioners empower individuals, their families, carers and communities to lead fulfilling, independent lives. Using our Let’s Talk Together strengths-based approach and Making Safeguarding Personal you will work collaboratively with people with social care needs to formulate support plans that maximise abilities, enabling individuals to make informed choices and work towards their identified goal.
Main duties:
- work with people in crisis, promoting, enabling and empowering individuals, families and carers through representation of rights; advocacy; nurturing strengths and capacities to make informed choices about highly complex life decisions.
- Network and liaise with other professionals and external organisations to provide services to individuals, families and carers and to challenge and question when necessary in the best interests of the user.
- Support people to receive support and treatment in the community rather than inpatient settings and to arrange effective discharge pathways, facilitating community connections, housing and discharge arrangements.
- Use active listening skills, observation and communication to develop supportive and therapeutic relationships with adults, their families and informal networks, exploring their situation with them and looking together for ways to move forward.
- Support and constructively challenge them to think about the social factors within their lives and help them establish their own goals for positive change, providing people with advice and the tools to achieve their aspirations.
- Refer, liaise, consult and work effectively with all relevant agencies and other professionals in order to help meet identified needs, bridging the gap between health and social care.
- Provide professional support and information, guidance and advice on how their needs could be partly or wholly met by universal and other non-care services.
- Work closely with the person or their carer to develop an agreed care and support plan to achieve the identified outcomes, promoting the delivery of strengths-based approaches to support planning, maximising on an individual’s strengths and community / family resources.
- Ensure care and support plans are person centred, capture the person voice and promote the independence, overall safety and well-being of adults and where relevant children.

Requirements of the Adults Safeguarding Practitioner:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant frontline experience in Adults Services within the UK
