We’re looking for a passionate and experienced Social Worker to join the Learning Disabilities and Autism Team in Hounslow. Your Responsibilities:

  • Undertake person-centred assessments to identify the needs and goals of individuals with learning disabilities and autism.

  • Develop, implement, and review care and support plans to promote independence and community integration.

  • Work collaboratively with service users, families, carers, and healthcare professionals to ensure joined-up care.

  • Provide advocacy and emotional support, empowering individuals to make informed decisions.

  • Maintain up-to-date records, adhering to legal and policy standards and contributing to service improvement.
