Duties:
- Plan, provide and evaluate a range of person centred social work interventions to individuals and their circle of support enabling individuals needs to be met and aspirations achieved
- Effectively manage a restricted case load with a complexity commensurate with the level of AYSE competence
- Provide a statutory range of assessments, reviews and reassessments using social work methods, theories and models to identify actions to achieve change and development, and that improve life opportunities
- Work within all relevant legislative requirements, policy and procedures to assess and meet the needs of adults and their circle of support and to provide appropriate support through the offer of a personal budget where appropriate
- Maintain accurate, comprehensive and comprehensible records in accordance with applicable legislation, protocols and guidelines
- Arrange and chair planning meetings, customer reviews and other ad hoc meetings as required
- Work closely with partners and other agencies in assessing, planning, arranging and co-ordinating a range of innovative services to meet the needs of adults and their circle of support, ensuring best value and qualitative arrangements are in place
- If required and as directed undertake duties within Home First responding to initial referrals and implementing short-terms pieces of work to prevent a need for longer term social work interventions
- Attend multi-disciplinary and multi-agency forums, promoting effective cross agency working practices and recognising diversity
- With appropriate support and guidance participate in safeguarding enquiries as required including liaising with multi-agency partners, taking appropriate action within legislation and reporting to stakeholders as appropriate
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Be flexible in undertaking the duties and responsibilities attached to their post and may be asked to perform other duties, which reasonably correspond to the general character of the post and are commensurate with its level of responsibility
- The ability to evidence continuing professional development
- Experience of Social Work practice
- Experience of safeguarding vulnerable adults
- Ability to act as a safeguarding investigator
- Comprehensive knowledge of current national and local drivers, policies and legislation including The Care Act, Mental Capacity Act, Deprivation of Liberties and Safeguarding Vulnerable Adults
- Excellent communication skills both verbal and written
- Ability to undertake a range of assessments and care planning for the service user group, taking account of quality and cost
- Able to work as a member of a multi-disciplinary team
- Good IT skills and ability to use IT systems in relation to electronic record keeping, correspondence and other related tasks
- Ability to research and analyse information and compile and present reports
- Good negotiation, influencing and networking skills
- Ability to develop and maintain successful professional relationships
- Tenacious and resilient
- Flexible
- Ability to transport self around North Lincolnshire in a timely manner
