Global Healthcare Jobs, a dedicated and award-winning recruitment agency looking for an experienced and dedicated Community Nurse to work in Isle of Wight on a full-time basis.

The salary for this permanent Community Nurse job is £35,392 – £42,618 per annum.

Main Duties:

  • To observe confidentiality of all information, adhering to the legal requirements of the Data Protection Act and Trust information sharing policies.
  • Accurately maintain written and electronic records, audits, work returns and statistics, submitting data and claim forms as required and within specified time-scales
  • Communicate with relatives/carers of patients, offering support and guidance as necessary.
  • In the absence of the Team Leader prepare, arrange and lead team meetings for the giving and receiving of patient reports and other relevant information.
  • Participate in team meetings and other professional meetings/groups as requested.
  • Liaise with the General Practitioner, nursing team and others in the planning and implementation of the care of the person, ensuring that all nursing actions are
  • recorded, including medical instructions and carried out effectively.
  • Attend Locality Multidisciplinary Team meetings.
  • When required, dissemination of information throughout the multidisciplinary team, taking into account the need for confidentiality.
  • Liaise with members of the Primary Healthcare Team, statutory and voluntary services as required, maintaining an effective standard of communication with all personnel
  • Work in partnership to integrate seamless care delivery across all care settings, preventing duplication, and fragmentation and delay occurring as patients move between care settings.
  • Ensure effective communication systems are in place to support care programmes and care pathways.
  • Enable and support integration of personal wishes into care planning for the present and future care needs.
  • Deliver and implement joint assessment and care plans.
  • Participate as a key member of the multidisciplinary team through the development of collaborative and innovative practice

Requirements of the Community Nurse:

  • Registered nurse with current NMC registration
  • Recent experience in community nursing
  • Willingness to undertake study at Level 6 or 7
  • Knowledge of the principles of manual handling and blood borne disease
  • Full UK driving liecence and access to own vehicle
