Location: Bromley

Team/Specialism: Assessment & Care Management

Pay Rate: £32 per hour (assignment rate)

Contract: Locum, 3-Months

Hours: Full-time. You may also be required to work weekends/bank holidays.

Duties:

  • Complete assessments under the Care Act 2014 and Mental Capacity Act 2002
  • Arranging care support to enable adults to return home to reduce the risk of social admissions to hospital or temporary placements to meet initial needs until further assessments can take place
  • Attend board meeting to discuss patient flow and plan the discharge
  • Supporting Younger Adults with physical disabilities and sensory impairment and Older People to live as independently as they are able and work with their carers to ensure risks are appropriately managed
  • Working within a multidisciplinary framework and need to have good communication and negotiating skills
  • Working with Partner agencies and will be required to develop close working relationships with them

Requirements of the Care Manager:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • An experienced locum social worker to work closely with Health colleagues to facilitate a safe return home for Vulnerable adults who present in the Emergency Department 
  • Have a knowledge/experience of facilitating safe Hospital discharges, strong working knowledge of the Care Act 2014 and its application including Section 42 Safeguarding threshold
  • Be flexible, committed, and hard working with a willingness to work to quality standards
