Global Healthcare Jobs, an innovative and committed recruitment agency has a new permanent position available for a Adult Social Worker within the Learning Disability & Preparation for Adulthood Team to work full time based in Hillingdon.
The salary for this permanent Social Worker job is up to £39,564 per annum.
Main duties:
- To provide a social work service to adults living Hillingdon. To carry out assessment using the agreed standardised format. To participate in co-ordinating services for people wishing to remain within their own homes. To offer support to people who may be moving into care.
- To work in a flexible way, adapting the service to meet the changing needs of service users from a wide range of social and cultural backgrounds.
- To work in an identified social work team within the Integrated Adult Community Care Service.
- To form relationships with service users to formulate an accurate assessment of complex psychosocial needs of service users, their families and service providers and access for services in line with local policy.
- To contribute towards a holistic assessment within the multi-professional specialist team, recognising that diversity, values and human rights may influence the decisions of people who use the service.
- To provide complex psychosocial support for service users with terminal illness/life threatening illness and their families, advocating for service users, carers, and families around complex emotional & practical needs.
- To provide specialist social work advice within the Adult Community Health & Social Care Team.
- To take a key role in responding to referrals to the Adult Social Work team carrying a caseload of adults in need.
- To respect an individuals independence and dignity and to offer person-centred support in making major life decisions and choices.
- To offer support, information & signposting to service users, families and carers on matters such as housing, benefits and finances, respite care and other community services, care home placements & third/private sector services.
- Assess, service users, carers, and family’s psychosocial concerns taking into account cultural, spiritual, psychosocial, social and financial issues and risk evaluation, in partnership with other members of the multi-professional team as appropriate.
- Assess family’s emotional resilience, coping skills, strengths, and difficulties within this practice setting, using expert social work skills & discussion with the multi-professional team.
- Provide emotional and psychosocial support to service users and their relatives, using individual, group, and family techniques with an aim to improve communication within the family/professional network and help to resolve difficulties. Referring those in need of formalised counselling to the psychosocial.
Requirements of this Social Worker job:
- A social Work Qualification or equivalent.
- Understanding of relevant legislation.
- Continual professional development.
- Social Work England registered.
