Key Responsibilities

  • Clinical Care Delivery: Administer intramuscular injections and conduct comprehensive assessments for individuals referred for ongoing mental health support.

  • Community-Based Interventions: Provide holistic care by visiting patients in their homes and across various community sites throughout the county.

  • Care Planning: Plan, implement, and review person-centred interventions to address clinical needs and effectively manage risks.

  • Multidisciplinary Collaboration: Participate in case conferences and reviews, actively involving service users and carers in the care planning process.

  • Caseload Management: Take accountability for managing delegated caseloads, including complex cases, and provide mentorship to junior team members.

  • Duty Cover: Deliver responsive support through telephone and face-to-face duty cover, conducting timely risk assessments and ensuring appropriate interventions are in place.
