Job Description
Global Healthcare Jobs, an innovative and committed recruitment agency has a new permanent position available for a Field Care Supervisor to work full time based in Chiswick, London.
The salary for this permanent job is up to £30,000 per annum.
Main duties:
- Work as part of the care management team managing a caseload of clients under the supervision of the Registered Manager
- Review risk assessments, update care plans, carry out care worker observations, client care visits and client quality checks
- Liaise with care workers, clients, their families or representatives, and community services to contribute to the delivery of effective and well-managed care
- Setting and reinforcing the high standard of care we expect from all our employees
- Use our digital care planning software on mobile and tablet devices and communicate effectively with colleagues and clients alike to ensure all duties are completed accurately and efficiently
- Client care visits
- Care worker observations and spot checks in the workplace
- Care worker supervisions
- Client quality checks
- Support the training of new care workers and the training of existing staff
- Provide support to care workers as required
- Manage the delivery of effective care to a case load of clients
- Maintain full and complete knowledge of the clients on your case load
- Maintain up- to-date care plans, risk assessments and client information
- Attend meetings with clients, their families and/or representatives as required
- Coordinate with local community teams (e.g. GPs, Pharmacies, District Nurses, OTs) to support holistic care
- Report issues and concerns to the Registered Manager
- Full knowledge and understanding of the Company policies and procedures
- Attend staff meetings and mandatory induction and on-going training
- Support the Registered Manager with ad hoc duties as required
- On call duties: minimum of one weekend each month and one evening each week
Requirements of this job:
- NVQ Level 3 in Health and Social Care (or currently working towards)
- Experience working in the community as a care assistant
- Experience working in a similar supervisory role
- Experience working with clients, in particular, those that may have additional support needs
- Good written and verbal communication skills
