Global Healthcare Jobs, an innovative and committed recruitment agency has a new permanent position available for a Social Worker within the Adult Care Management Team to work full time based in Waltham Forest.

The salary for this permanent Social Worker job is up to £45,815 per annum.

Main duties:

  • Conduct assessments of vulnerable people’s circumstances and issues to determine intervention/referral to the appropriate service.
  • Plan and implement interventions and action for allocated cases. Monitor and review cases. Assess in more complex cases, with guidance.
  • Provide professional advice and guidance to colleagues and partner agencies supporting vulnerable people and carers are supported.
  • Maintain properly documented case files, records and information. Prepare standard reports as required.
  • Carry out all duties with an awareness and understanding of the safeguarding requirements within area of responsibility.
  • Provide advice and support to vulnerable people to assist in completing financial assessments.
  • Proactively develop professional knowledge, skills and behaviours.
  • Act in accordance with all policies and procedures which apply to the job and understand the reasons for this.
  • All opportunities to prevent, reduce and delay access to social care services are taken.
  • Maximising residents ability to maintain their independence.
  • To be responsible for ensuring the delivery of high quality and legally compliant social care practice to agreed standards for the staff within the remit of the post.
  • Act as care coordinator under the Care Programme Approach (CPA) and care management process of assessment, commissioning and monitoring.
  • To ensure the implementation of Making Safeguarding Personal.
  • To ensure that the assessment and support planning process is robust and meets agreed guidance and standards.

Requirements of this role:

  • Social work qualification with registration with Social Work England.
  • Experience in managing social work caseloads and supervising others.
  • Experience of community and/or group work.
  • Knowledge of children’s health and social care systems, agencies, and relevant legislation.
