This ongoing, full-time role offers a dynamic work schedule of 36 hours per week, Monday to Friday, from 9 to 5. What you will do:

• Complete social care needs assessments with individuals to determine appropriate support and services.
• Carry out support planning, commissioning, and risk assessments to manage and mitigate risks effectively.
• Engage in safeguarding processes, including Mental Capacity Assessments/Best Interests decisions, and Continuing Healthcare – Decision Support Tool (DST).
• Attend Multi-Agency Risk Assessment Meetings (MARAMS) and other relevant training and duty days.
• Collaborate with a dedicated team, requiring office presence on Wednesdays and Fridays for vital interdisciplinary work.
