Pay Rate: £30 per hour (assignment rate)

Main duties:

  • Conduct Care Act assessments of individuals who have been discharged from the hospital and require community support.
  • Assess and evaluate the individual’s physical, mental, and emotional needs, as well as their functional abilities, to determine the appropriate level of care and support required.
  • Collaborate with individuals, their families, and other healthcare professionals to gather relevant information and develop comprehensive care plans.
  • Provide advice, guidance, and emotional support to individuals and their families, helping them to understand their options and make informed decisions.
  • Liaise with relevant community resources, such as healthcare providers and voluntary organisations, to ensure the seamless provision of services.
  • Advocate for individuals’ rights and entitlements, ensuring that they receive appropriate and timely support services.
  • Maintain accurate and up-to-date records of assessments, interventions, and outcomes in accordance with organizational and legal requirements.
  • Stay current with changes in legislation, policies, and best practices related to social work, care assessments, and community support services.

Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services within the UK
  • Valid driver’s license and own car with business insurance
  • Proven experience in conducting assessments and developing care plans.
  • Experience in Discharge to Assess would be an advantage but not essential.
  • Sound knowledge of the Care Act, Mental Capacity Act and Best Interest Processes is essential for this role.
  • Knowledge and experience of Continuing Health Care processes and participating in DST’s would be an advantage, but not essential.
  • Ability to work in a fast paced role is essential for this role.
