Pay Rate: £20.68 per hour (assignment rate)

Contract: Locum, 3-Months

Duties:

  • Maximise the independence and safety of the vulnerable peopleof North Yorkshire who have care needs
  • Undertaking assessments of need with vulnerable peopleusing the Directorate’s procedures, and where required you will undertake separate assessments of the needs of the person’scarer(s)
  • Support vulnerable peopleand carers who have been assessed as being in need to develop appropriate care packages to meet the person’s or carer’s preferred outcomes within specified resources or by enabling peopleto access universal community resources
  • Assist the Directorate in meeting its aim of closer working relationships with Health and other partners to deliver improved outcomes for vulnerable adults in North Yorkshire
  • Work at all times in line with the Health and Care Profession Councils Code of Practice

Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • This role involves spoken communications so a confident use of English language is required
  • Be aware of and implement your health and safety responsibilities as an employee and where appropriate any additional specialist or managerial health and safety responsibilities as defined in the Health and Safety policy and procedure
  • Good working knowledge, understanding and application of community care legislation and regulations
  • Knowledge and understanding of social care policy developments and good practice at a local and national level
  • Knowledge of statutory requirements, including requirements in respect of carers, equality and anti discrimination legislation, maintaining a safe working environment, data protection and confidentiality
  • Knowledge and understanding of how Equality & Diversity, Dignity & Respect and Human Rights will apply to this role
  • Experience of undertaking assessment of health or social care needs in a community setting
  • Experience of inter-agency collaboration practice
  • Demonstrable experience of undertaking complex assessment of health or social care needs in a community setting
  • Demonstrable experience of initiating the appropriate statutory and other actions required to undertake adult protection investigations
  • Experience of working positively in a changing environment
  • Relevant community experience for nursing qualified applicants
