Pay Rate: £40 per hour (assignment rate)
Contract: Locum, 3-Months

Main Duties:
- To undertake and/or co-ordinate assessments for adults and young people coming into Adult Services using Personal Outcome Planning tools in partnership with service users, carers and colleagues in other agencies.
- To develop care packages in partnership with the above.
- To undertake care management for adults to ensure that care packages continue to meet the needs of service users and their carers.
- To record all activities and utilise the departments Carefirst information system.
- To liaise with Community Care Managers in relation to commissioning of new services to meet the assessed need of individuals.
- To liaise with service providers as appropriate.
- To be part of the out of hours rota as appropriate.
- To participate in supervision to allow monitoring and development of performance.
- To travel efficiently and effectively between various work locations within Argyll and Bute and beyond to meet the operational requirements of the Service.
- To undertake training as required.
- To record work as requested by departmental procedures.
- To carry out any other duties appropriate to the grade of the post as may be required
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant frontline experience in Adults Services within the UK
