Pay Rate: £30 per hour (assignment rate)

Summary/Main duties:
- This is a specialist role, with a focus on dysphagia management, to work within our well established DIST (Dementia Intensive Support Team), with a focus on admission avoidance; discharge planning, community and in-patient assessment and management, to individuals under the care of or referred to the service. The role will also work with the team to provide ad hoc training and support to carers in their roles in all environments.
- The post holder will work effectively and efficiently as an autonomous speech and language practitioner within a multi-professional team environment in community, primary care and acute settings, ensuring safe, efficient and effective delivery of patient focussed care in line with identified evidence based practice, local and national standards.
- Independently assess, differentially diagnose and develop and implement specialist care programmes for patients, open to the service, with swallowing needs.Work with the multidisciplinary team to ensure a coordinated care plan for patients and necessary support and follow up from this care plan.
- Liaise and work closely with patients, carers and families to establish goals for therapy/management
- Have and maintain specialist competencies and skills in dysphagia.
- Contribute to the development and delivery of dysphagia training programmes to members of the multidisciplinary team and wider system partners.
- Demonstrate critical thinking, reflection and analysis to inform practice
- Be up to date with relevant local and national standards and to lead in the implementation of evidence based practice.
- Develop links with other Speech and Language Therapy Departments in other Trusts and localities
Requirements of the Speech and Language Therapist/SLT:
- Degree or equivalent within Speech and Language Therapy and HCPC registration
- Recent UK/NHS post qualifying experience working as a Speech and Language Therapist
- Car driver
