Banding/Grade: Band 5
Pay Rate: £25.00 per hour (assignment rate)
Job Type & Hours: Full-Time, Monday-Friday, Locum

Job Duties:
- Provide high-quality nursing care to individuals within the community setting.
- Conduct comprehensive assessments and develop care plans based on individual needs.
- Administer medications, perform wound care, and assist with various treatments as required.
- Collaborate with healthcare professionals and multidisciplinary teams to ensure coordinated care delivery.
- Educate patients and their families on health management and self-care techniques.
- Monitor patient progress, evaluate outcomes, and make necessary adjustments to care plans.
- Maintain accurate and up-to-date documentation of patient interactions and interventions.
Requirements of the Community Nurse:
- RGN qualified/NMC registered with valid live pin
- Recent UK nursing experience
- Must have Community/District nursing experience
- Proven experience as a community nurse or in a similar role.
- Strong clinical skills and knowledge of community healthcare practices.
- Excellent communication and interpersonal abilities.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Strong organizational and time management skills.
