Global Healthcare Jobs, a dedicated and award-winning recruitment agency looking for an experienced and dedicated Community Mental Health Nurse to work in Guernsey on a full-time basis.
The salary for this permanent Community Mental Health Nurse job is £34,876 – £44,981 per annum.
Main Duties:
- To undertake the assessment of patients and clients with complex care needs including individuals with undifferentiated and undiagnosed conditions.
- To develop, implement and evaluate programmes of care to meet these needs.
- To guide, advise and direct other staff in the assessment, planning, implementation and evaluation of patient and client care.
- To initiate risk assessment processes to determine risks to the health and well-being of patients/clients with complex care needs, taking relevant action to minimise these risks.
- Under the supervision of the Team Leader to lead the team in emergency and other acute situations, ensuring that the patient’s/client’s needs are met and that significant others are supported.
- To utilise information and data from a range of sources to identify patient/client problems and guide decision-making processes. Within field of expertise, and organisational policy, request specific clinical tests to establish a diagnosis.
- To promote service user involvement in the planning, delivery and evaluation of care, respecting their wishes, beliefs and dignity.
- To independently liaise and communicate effectively with all member of the multidisciplinary team and other agencies involved in the care of the patient/client.
- Manage a full caseload and co-ordinate care for their service users.
- Be responsible for the assessment and care management of service users in a community setting.
- Carry out comprehensive risk assessment and management plans for service users in a community setting.
- Liaise with other health care professionals including, Psychiatrists, social workers, GP’s, Probation, Prison staff, Children’s services, Older adult services etc.
- Attend ward reviews as required.
- Attend child protection case conferences, MARAC, MAPPA as required.
- Supervise and support service user’s transition between services.
- Supervise and mentor junior staff and students on placement with an expectation mentorship qualification is completed before progression to band 6.
- Attend ward reviews as required.
- Keep accurate electronic and paper records in accordance with local policy and guidelines.
- Maintain standards of practice, including, audit and the collection of accurate data.
- Adhere to HSC policy and NMC professional guidelines.
- Actively promote evidence based practice.
- Maintain essential skills and continuing professional development relevant to the post.
Requirements of the Community Mental Health Nurse:
- Registered Mental Health Nurse
- Evidence of CPD at diploma level
- Evidence of Working with a wide range of Professionals, and ability to work Autonomously & Independently
- Demonstrate a good level of IT skills.
- Current driving licence
